SSM Health Sports Medicine is offering FREE Sports Physicals for our partnered High Schools. You MUST sign up on the link below in order to receive a physical.

Please sign up for one 10 minute appointment slot, but please allow up to 45 minutes for your appointment. Be sure to sign up with the child receiving the physicals name, not the parents name. Please note there are several dates and locations available.
